Autoimmune polyendocrine syndrome is a rare syndrome with Addison’s disease and thyroiditis as its major components, with the former mandatory for diagnosis. We report a 27 year old woman with primary infertility who had subclinical hypothyroidism, normal LH/FSH and elevated prolactin. Following thyroxine supplementation, she developed adrenal crisis. Investigations revealed Addison’s disease, autoimmune thyroiditis and ANA positivity. She did well with prednisolone and thyroxine supplementation, but was lost to follow-up.
